Before we meet, there are three quick things to take care of. They'll take about 10–15 minutes and make a real difference in how smoothly our visit goes.


Three Things to Do Before Your Visit

Complete Your Pre-Visit Forms

After scheduling, you should have received 3–4 forms by email with the subject: “Fill out forms for OurEra Health”

Please complete these as soon as possible; they are needed to set up your profile in my electronic health record before we meet. The forms are secure, confidential, and as brief as possible.

Check your inbox and spam folder if you haven't seen them yet.

Connect to Spruce: Our Secure Messaging Service

Spruce messaging is always the preferred way to contact me.

Even before your very first visit, be sure to connect to my practice in Spruce so we can communicate if you have any questions or difficulty getting set up.

Register for Elation Patient Passport

After scheduling, you should have received an email invitation with the subject: “Invitation to Elation Passport - Your Secure Patient Portal.”

This is your secure portal to access your medical records and manage appointments.

Once you’ve registered, use this link to login. → app.elationpassport.com/passport/login/


At the Time of Your Visit

Use any device (desktop, laptop, phone) to click the Zoom link you received, and it will take you to my virtual waiting room. Plan to join a few minutes early in case you need to configure Zoom for your system.

If you have difficulty joining, use Spruce to let me know.

If you’ve not been able to set up Spruce, you can also text (510) 462-5894 using the phone number you provided when scheduling your appointment, and we’ll work through any problems.

I’ll join at the time of the visit, and we’ll have up to an hour to talk and develop a care plan to meet your treatment goals.
