You’re eligible to enroll if you can answer “yes” to these 3 questions:
Are you a California resident who is at least 16 years-old?
Do you have a problem or concern related to opioid or alcohol use?
(opioids include prescription pain medication, kratom, 7-OH, heroin, and fentanyl, among others.)
Is your insurance plan on this list? (click + for plan types)
Note: If your plan type is an HMO or MCO, then I cannot accept your insurance plan.

Schedule Your Visit
You’ll receive a scheduling link immediately after you connect to my practice in Spruce.

Submit Your Basic Health Information, Insurance Information, and Consent Forms
After scheduling, you’ll receive 4 forms by email with the subject: “Fill out forms for OurEra Health”
→ Check your inbox and spam folder if you haven't seen them yet.
The forms are secure and confidential. All-together, they take about 15 minutes to complete. Please complete them as soon as possible.
At the Time of Your Visit
Use any device (desktop, laptop, phone) to click the Zoom link you received, and it will take you to my virtual waiting room. Plan to join a few minutes early in case you need to configure Zoom for your system.
If you have difficulty joining, use Spruce to let me know.
If you don’t have access to Spruce, you can also text (510) 948-7858 using the phone number you provided when scheduling your appointment, and we’ll work through any problems.
I’ll join at the time of the visit, and we’ll have up to an hour to talk and develop a care plan to meet your treatment goals.
