How it works

  1. Schedule Your Visit

Request your initial visit by completing our secure form. It only takes about 3 minutes, then you’ll receive scheduling information by SMS or email. Initial visits can often be scheduled within just 1 day of the request. You’ll receive a consent form and health information questionnaire after scheduling.

Your enrollment and your form responses are confidential and treated as protected health information.

2. Meet Your Provider, Dr. Brian Clear, by Video

Complete your initial video consultation with Dr. Clear.

There’s no wait list and no long pre-visit intake process. You’ll meet Dr. Clear online using your personal computer or smartphone for a telemedicine assessment that will be billed to your insurance afterward.

The initial visit can take anywhere from 20 minutes to an hour, depending on your needs and availability.

3. Begin Your Care Plan

You and Dr. Clear will develop your care plan during the visit, and afterward you’ll receive a care summary, and prescriptions will be sent to the pharmacy of your choice. The most commonly prescribed medication for opioid addiction is buprenorphine (Suboxone, Sublocade, Brixadi)

If labs are ordered, you can complete them at any Labcorp location statewide.

We work with partner programs to provide coordinated coaching, therapy, and contingency management if your care plan includes these services.

You’ll continue to follow-up with Dr. Clear regularly, on a schedule that considers your preferences and availability.

Ourera is Dr. Clear’s private practice, and he is personally responsible for every patient’s care. He is licensed by the California Medical Board, and is board certified in Addiction Medicine, and Family Medicine. He is a Fellow in the American Society of Addiction Medicine, and he is the founding Chief Medical Officer of Bicycle Health Medical Group.

FAQs

What conditions does Ourera treat?

We care for patients, age 16+, anywhere in California with active or past addiction to opioids including kratom, 7-OH mitragynine, pain medication, fentanyl, and heroin.

We provide medications for Opioid Use Disorder itself, and we can also treat co-occurring substance use disorder(s) such as Alcohol, Stimulant, or Sedative/Benzodiazepine Dependence. We can also provide treatment for some co-occurring mental or general medical conditions, like depression, when needed. Ourera does not provide in-person primary care.

Will telemedicine work for me?

Care for most patients with a substance use disorder (addiction) can be provided effectively through telemedicine. You just need consistent access to a smart device with internet access and video-call capability, whether it’s your phone, tablet, laptop, or desktop. If you’re unsure about telemedicine, we can talk about the benefits and risks in more detail at an initial visit and make an informed decision together.

How does it work?

Enroll Now to set up your initial visit. You’ll receive a link to our secure enrollment forms to complete. All information you provide is considered protected health information and is kept confidential as per our Notice of Privacy Practices.

At the visit, you’ll meet Dr. Clear by secure video conference, discuss your care goals, align on a plan, then receive treatment recommendations and start care at the same visit. You’ll schedule follow-up and learn how to contact us for sooner care as needed. You’ll fill any prescribed medication at your chosen pharmacy.

Will Ourera be my primary care provider?

It’s preferred for most patients to also have a separate primary care provider (PCP) with an in-person office available. With your consent, Ourera can share records and coordinate with your primary care provider.

While Ourera will not be your official PCP, we can provide limited primary care services through telemedicine like treatment of mood disorders, high blood pressure, and some other chronic condition management when needed. Our service is not effective for emergencies or acute care.

Do you provide medication?

Yes, we prescribe medications that are safe and effective for your presenting condition(s). For Opioid Use Disorder, the most commonly prescribed medication is buprenorphine, which is the active ingredient in Suboxone, Sublocade, Brixadi, and Zubsolv.

In addition to treatment of substance use disorders, we also may prescribe medications to treat other related conditions.

Do you take my insurance?

We are in-network with Aetna, Anthem Blue Cross, Blue Shield of California, UnitedHealthcare (UHC) / Optum, and Medicare Part B throughout California.

We are working toward accepting Medi-Cal in Alameda County from late 2026.

Is there a self-pay option?

There is. As long as you don’t have active Medi-Cal, any uninsured patient or any patient who does not wish to use insurance may choose to self-pay for services.

Is my care confidential?

Yes. From the time you submit a new patient request, your identity and all health information you provide us are considered protected health information and shared only as described in our Notice of Privacy Practices. We encourage coordinated care with your other providers, and we also respect that your medical record exists only for your benefit, and you’re in control.
